To make the most efficient use of your appointment time, please complete the forms in this section, and email them to me before your first visit. Private patients are welcome to send an email requesting my current fees.
For informational purposes only, a link to the federal Centers for Medicare and Medicaid Services (CMS) Open Payments web page is provided here. The federal Physician Payments Sunshine Act requires that detailed information about payment and other payments of value worth over ten dollars ($10) from manufacturers of drugs, medical devices, and biologics to physicians and teaching hospitals be made available to the public.
Physicians are required to notify their patients of the existence of the Open Payments Database which lists all payments made by pharmaceutical companies to doctors over the past several years. To access this database go to: https://openpaymentsdata.cms.gov
